Parent/Guardian Consent & Liability Waiver

Please read the following carefully and sign below. If anything feels unclear, you are always welcome to ask questions before you sign.

1. What is The Happy Core™ Program?

The Happy Core™ Program is an educational and coaching-based program

that teaches children:

- Emotional awareness and regulation

- Coping skills for big feelings

- Tools for focus, self-confidence, and resilience

- Positive communication and problem-solving

Sessions may include: - Simple movement and “brain gym” exercises

- Breathing techniques and relaxation tools

- EFT/Tapping or Havening-style calming touch (self-applied or taught with guidance)

- Guided visualizations, child-friendly hypnosis or mutual stories

- Creative activities and games

- Group discussions and reflections

2. Nature of Services (Not Therapy or Medical Treatment)

I understand that:

The Happy Core™ Program is not medical treatment, psychotherapy, or a substitute for professional mental health or medical care.

No diagnosis or medication will be prescribed, changed, or discontinued as part of this program.

If my child has a diagnosed mental health condition or medical condition, their primary care provider, pediatrician, or mental health professional remains responsible for their treatment.

The Happy Core™ is a skills-based, educational and coaching program that provides tools and strategies to support emotional and everyday functioning.

3. Participation & Responsibilities

I understand that:

Participation in The Happy Core™ Program is voluntary. My child may choose not to participate in a particular activity and may take a short break if needed.

I remain responsible for my child’s overall well-being, decisions, and follow-up outside of sessions.

For best results, I am encouraged to:

- Support and encourage the use of tools at home,

- Communicate openly with Kristin about any concerns, and

- Inform Kristin of any significant changes in my child’s health, medication, or emotional state.

4. Group Setting & Confidentiality

In case your child participates in a group setting of The Happy Core™ Program (school group, small group class, workshop).

In this case, I understand: Kristin will encourage a respectful, kind, and confidential environment, but complete confidentiality cannot be guaranteed in a group of children.

Children may share feelings or experiences aloud in group activities.

General progress or themes (not private details) may be shared with parents and, where relevant, school staff to support the child.

5. Online Sessions

(If your child's Happy Core session is online, for example via Zoom or a similar platform:)

You agree that your child will join the session from a quiet, private space where they will not be disturbed.

You understand that technical issues (internet, audio, video) may occasionally interrupt the session and we will do our best to reconnect or reschedule if needed.

6. Health, Emotional Safety & Suitability

I understand that: Some activities may bring up feelings, memories, or emotional reactions in my child. This is a normal part of learning emotional skills, and Kristin will guide my child gently and supportively.

If my child has a history of:
- Epilepsy or seizures
- Severe trauma
- Self-harm or suicidal thoughts
- Psychosis, severe mood disorder, or other serious mental health conditions
I will inform Kristin prior to the start of the program so that she can decide whether The Happy Core™ is appropriate at this time or if adjustments are needed.

The Happy Core™ Program is not a crisis service. In an emergency or acute mental health situation, I will contact emergency services or my child’s healthcare provider.

7. Movement & Touch-Based Techniques

I understand that: Sessions may include gentle movement, stretching, and body-based tools (e.g., “brain gym” exercises, tapping on acupressure points, or calming touch such as self-havening).

My child will be invited, not forced, to participate and activities are designed to be age-appropriate and low-impact.

Any touch-based tools will be:
- Demonstrated for self-use, or
- Only used with clear explanation and respect for personal boundaries.

If my child has physical limitations, injuries, or medical conditions that affect movement or touch sensitivity, I will inform Kristin in advance.

8. Communication with School / Other Providers (If Applicable)

If The Happy Core™ is offered through a school or organization, I understand that:

- With my consent, Kristin may share general observations or strategies with teachers or support staff to help my child apply the tools in everyday situations.

- I may revoke or limit this permission at any time in writing.

9. Media, Photos & Recordings

9.1 - Personal Photos / “Hey Mom, look what I’m doing” Updates Sometimes we do special activities (for example, going to the beach, creative projects, or fun exercises), and I love being able to share a little moment of joy with you.

Please choose one:

Untitled checkboxes field
I understand that: These photos are sent only to me (or the contact number I provide).
They are intended as a simple “look what we did today” update, not for public use.
I can change my preference at any time by informing Kristin in writing.

9.2 - Media & Recordings for Program Improvement or Promotional Purposes

Separately from personal updates, The Happy Core™ may occasionally take photos or short video recordings for: - Program documentation and improvement, and/or - Website, social media, or printed materials to show what the program looks like. I understand that:

- My child’s image will not be used for promotional or public purposes without my separate, explicit written consent.

- If such use is planned, I will be asked to sign a separate media consent form where I can opt in or out.

- My child’s privacy will be respected and no sensitive personal information will be shared.

10. Booking, Payment & Cancellation (Happy Core™ Programs)

The full details are outlined in the current Booking & Cancellation Policy, which I have read and agree to: https://www.transformationtothecore.com/the-happy-core-booking-policy.

10.1 - Booking & Payment I understand that:

- My child’s place in The Happy Core™ Program / Homework Hero Workshop is reserved specifically for them once their registration is confirmed and payment has been received, in line with the Booking & Cancellation Policy. - Group and after-school spots are limited, and holding a place for my child means that this spot is not available to another family.
- Program and workshop fees are communicated in advance, and payment is due before the start of the program or workshop, unless a different arrangement has been agreed in writing.

10.2 - Cancellation & Group Fit

I understand that:

If I cancel my child’s participation more than 7 days before the program or workshop start date, any fees already paid may be refunded or credited toward a future Happy Core™ or Homework Hero™ offering, at Kristin’s discretion.

If I cancel my child’s participation within 7 days of the start date or after the program has begun, fees are generally non-refundable, as the space has been held for my child and preparation time has been invested.

Missed sessions (for example, due to illness, family events, or schedule conflicts) are not automatically refunded, and make-up sessions are only offered if available and cannot be guaranteed, as outlined in the Booking & Cancellation Policy. I also understand that: Many children in these programs have ADHD, anxiety, or learning differences, and it is normal for some days to be harder than others. My child will always be encouraged—but not forced—to participate and can take breaks when needed.

If my child is consistently very distressed, highly disruptive, or unwilling to participate over time in a way that affects their experience or the group, Kristin will contact me so we can discuss what is happening and explore adjustments or additional support.

If, after reasonable efforts and communication, it is decided that the group format is not the right fit for my child at this time, and my child is withdrawn from the program or workshop for this reason, remaining fees are generally not refundable, as the place has been held for my child and preparation time has taken place.
